We have your convenience in mind. With the NEW online patient registration portal you can fill out your patient registration any time of day. Registering online will save time when you arrive for your appointment. This will ensure that you are seen as quickly as possible when you arrive for your appointment. Registration only takes about 10 minutes. Before you start the registration process, have your medical and vision insurance cards at hand. Fill out the information as accurately and completely as possible. You must fill out all required fields for the registration process to be complete. Our practice accepts most major insurance plans and most of the smaller insurance plans. If we do not participate with your insurance and your plan allows out of network benefits, our office staff will assist you with the necessary paperwork so you can receive reimbursement directly from your insurance company.
If you do not see your insurance listed below, please call the office to speak with one of our knowledgeable staff members about your individual coverage.

To make the most of your first appointment, please try to arrange the following:
- Past psychiatric/psychological/neurological reports: send any relevant reports from previous MD or psychologists by FAX (516) 684-9536 or email to email@example.com. If patient is not able to communicate well because of memory or other problems, please have the patient accompanied by a family member or somebody who has known the patient in last few years.
- Your insurance card, copayment. it is recommended that you call your insurance company to find out if your deductible is met for the year and your copayment for mental health visit.
- The appropriate forms
APPOINTMENT RESCHEDULING/ CANCELLATIONS:
With the exception of serious emergencies (e.g., being in the hospital), it is expected that you keep all of your appointments. If you need to reschedule an appointment we require that you call at least one business day before your scheduled appointment. In such a case, please call our office to reschedule/cancel your appointment with one of our patient care coordinators ( before 5pm the day before the appointment). When you make an appointment you are reserved for that time slot with physician. This time then becomes your responsibility. Individuals missing their appointments, canceling or changing it with less than one business day period notification, will be charged $75 for that reserved appointment time. Law prohibits billing your insurance carrier for such charges and therefore you will be billed directly. Although there may be legitimate reasons for canceling any appointment, there is a need to maintain the integrity of the practice. Unfortunately this needs to be a blanket policy. Rescheduling/ cancellation for Monday’s appointment should be called in by Friday 5PM.

In case of emergencies, to reach physician after office hours, please leave a voice message on (516) 472-7450 and you will be contacted as soon as possible.
PRESCRIPTION RENEWAL BY PHONE:
Such requests are only acceptable in the event of emergencies, when you are unable to keep your anticipated appointment. In such cases, the medication can be renewed for a limited time period (1 to 2 weeks) to enable you to have enough medications until your next scheduled appointment. Prescriptions for controlled drugs (benzodiazepines, stimulants, sedatives, etc.) can be renewed for 5 days only (as per DEA regulations). Such prescriptions will incur a $15 fee for administrative charges (calling the pharmacy with the prescription number, getting the prescription from the MD, and mailing it to your pharmacy within the next 7 days).
